LabLookup Terms of Use

These Terms of Use are between you and Understand My Policy, Inc., a Delaware corporation, doing business as LabLookup (“LabLookup,” “we,” “us,” or “our”).
By using LabLookup, you agree to these Terms. If you do not agree, do not use LabLookup.
1. What LabLookup is
LabLookup is a specimen collection reference and workflow support tool. It helps users look up and organize lab test collection information such as tube type, specimen volume, lab order codes, draw order, storage temperature, and handling notes.
LabLookup is intended for informational and workflow reference purposes only.
2. What LabLookup is not
LabLookup is not:
- an electronic health record
- a lab ordering system
- a lab results viewer
- a diagnostic tool
- a clinical decision support system for diagnosis or treatment
- a substitute for professional medical judgment
- a substitute for official laboratory source materials, policies, or instructions
LabLookup does not order tests, transmit orders, interpret results, diagnose conditions, recommend treatments, or determine what tests should be ordered for a patient.
3. No patient information
Do not enter patient information into LabLookup.
You agree not to enter, upload, submit, store, or transmit any patient-identifying information, including:
- patient names
- dates of birth
- medical record numbers
- addresses
- phone numbers
- email addresses
- Social Security numbers
- insurance or member IDs
- diagnoses
- symptoms
- lab results
- medication lists
- clinical notes
- any other information that identifies or could reasonably identify a patient
LabLookup is designed to work without patient information. If you enter patient information despite this restriction, you are responsible for that use.
4. No PHI; not a HIPAA business associate
Because LabLookup is designed not to receive protected health information (PHI), LabLookup does not act as a HIPAA covered entity or business associate, and we do not enter into Business Associate Agreements. You are solely responsible for ensuring your use of LabLookup complies with HIPAA and any other privacy obligations that apply to you, including by not entering PHI.
5. Reference-only information
LabLookup may display information based on publicly available lab reference materials, clinician review, user feedback, or other sources.
We try to keep information accurate and useful, but lab requirements can change and may vary by lab, location, specimen type, patient context, collection method, or other circumstances.
You are responsible for verifying collection requirements against the relevant laboratory’s official source materials, policies, and instructions before relying on them.
6. Professional responsibility
LabLookup is intended to support qualified clinical and office staff, not replace their judgment. You are responsible for your own clinical, operational, and professional decisions.
You should not rely on LabLookup as the sole source of truth for any collection, storage, processing, or handling decision.
7. User corrections and feedback
LabLookup may allow users to flag possible errors, suggest corrections, or provide feedback.
Submitting feedback does not guarantee that LabLookup will make a change. We may review, edit, accept, reject, or remove feedback at our discretion.
Do not include patient information in feedback or correction submissions.
8. Accounts
Some features may require an account. You are responsible for maintaining the confidentiality of your account credentials and for activity under your account.
We may suspend or terminate access if we believe the product is being misused, used unlawfully, used to submit patient information, or used in a way that creates risk for LabLookup, users, patients, labs, or others.
9. Plans, billing, and cancellation
Certain LabLookup features are offered under paid plans. By starting a paid plan you authorize us (through our payment processor) to charge the applicable fees, including any recurring subscription fees, to your payment method. Unless stated otherwise, subscriptions renew automatically each billing period until cancelled, and you may cancel at any time effective at the end of the current period. Fees are non-refundable except where required by law. We may change plans or pricing on prospective notice. You are responsible for applicable taxes.
10. Acceptable use
You agree not to:
- enter patient information
- use LabLookup to provide diagnosis or treatment recommendations
- use LabLookup to order tests or transmit orders
- copy, scrape, harvest, or bulk extract LabLookup content
- interfere with the security or operation of the product
- use LabLookup for unlawful, misleading, harmful, or abusive purposes
- reverse engineer or attempt to access non-public systems or data
11. Third-party labs and sources
LabLookup may reference third-party laboratories, test directories, codes, or source materials. LabLookup is independent and is not endorsed by, sponsored by, or affiliated with Quest Diagnostics, Labcorp, BioReference, or any other laboratory unless expressly stated.
All third-party names, trademarks, and materials belong to their respective owners.
12. Intellectual property
LabLookup, including its design, interface, workflows, compiled reference format, text, software, and other content, is owned by Understand My Policy, Inc. or its licensors.
You may use LabLookup for your own internal reference and workflow purposes. You may not copy, reproduce, distribute, resell, or create a competing database or service from LabLookup content without written permission.
13. Disclaimers
LabLookup is provided “as is” and “as available.”
To the fullest extent permitted by law, we disclaim all warranties, express or implied, including warranties of accuracy, completeness, reliability, fitness for a particular purpose, merchantability, non-infringement, and uninterrupted availability.
We do not guarantee that LabLookup will be error-free, complete, current, or available at all times.
14. Limitation of liability
To the fullest extent permitted by law, Understand My Policy, Inc., LabLookup, and their owners, officers, employees, contractors, and agents will not be liable for indirect, incidental, consequential, special, exemplary, or punitive damages, or for lost profits, lost data, business interruption, patient care delays, redraws, rejected specimens, or other losses arising from use of LabLookup.
To the fullest extent permitted by law, our total liability for any claim relating to LabLookup will not exceed the greater of: (a) the amount you paid to use LabLookup in the three months before the claim arose, or (b) $100.
Some jurisdictions do not allow certain limitations, so some of the above may not apply.
15. Indemnification
You agree to defend, indemnify, and hold harmless Understand My Policy, Inc., LabLookup, and their owners, officers, employees, contractors, and agents from claims, damages, liabilities, costs, and expenses arising from your use of LabLookup, your violation of these Terms, your entry of patient information, or your misuse of the product.
16. Changes to LabLookup or these Terms
We may update LabLookup and these Terms from time to time. The updated Terms will be posted with a new “Last updated” date. Continued use of LabLookup after changes means you accept the updated Terms.
17. Governing law, disputes, and venue
These Terms are governed by the laws of the State of Delaware, without regard to conflict of law principles. The state and federal courts located in Delaware will have exclusive jurisdiction over any dispute arising out of or relating to these Terms or LabLookup, and you consent to personal jurisdiction and venue there.
18. General terms
If any provision of these Terms is found unenforceable, the remaining provisions stay in effect. These Terms are the entire agreement between you and us regarding LabLookup and supersede prior agreements on that subject. You may not assign these Terms without our consent; we may assign them in connection with a merger, acquisition, or sale of assets. Our failure to enforce a provision is not a waiver. We are not liable for delays or failures caused by events beyond our reasonable control.
